How would I put this into APA format for my bibliography?

I think thats it in a apa format, but i used my uni website which is really really useful, so double check on the website if this is right: http://library.curtin.edu.au/referencing/apa.pdf
if the link doesnt work go to www.curtin.edu.au then go to library, then referencing and then APA referencing in the left column. But yeh it has apa referencing for every format.
